How to configure the new style questionnaire

Click on the "New" button to create a new question

Add as many questions as you wish

(1) Select the type of question from the drop down option

(2) You can change the order of the questions with the "Move Up" and "Move Down"! buttons

(3) You can remove the questions with the "Delete" button

(4) Make sure you enter a unique fieldname for use with the Mail Merge

(5) You can edit further details of each question by clickin on the "Details" button

Drop Down Questions must have at least one answer added before they can be saved
